The Centers for Disease Control and Prevention (CDC) is investigating a concerning outbreak of botulism-like illnesses linked to counterfeit or mishandled botulinum toxin injections, commonly referred to as “Botox.” This investigation comes after reports of adverse reactions in individuals across multiple states.

What Is Happening?

As of January 2025, the CDC has identified numerous cases where patients experienced symptoms consistent with botulism after receiving botulinum toxin injections. These symptoms include:

blurred or double vision, drooping eyelids, difficulty swallowing, dry mouth, slurred speech, difficulty breathing, fatigue, and generalized muscle weakness.

Alarmingly, 76% of those affected required hospitalization, and 35% were treated with botulism antitoxin due to concerns about the toxin spreading beyond the injection site.

Why Is the CDC Investigating?

The CDC’s primary objectives are to determine the root cause of these illnesses and prevent further cases. The investigation is focused on identifying whether the outbreaks stem from counterfeit products, unregulated sources, or improper administration by unlicensed or untrained individuals. Additionally, the CDC is working to educate both healthcare professionals and the public about the risks associated with unauthorized or mishandled botulinum toxin.

Botulism is a rare but serious condition caused by a toxin that can lead to life-threatening complications, such as respiratory failure. Ensuring that these cases are thoroughly investigated is crucial for public safety.

What Does “Mishandled Botox” Mean?

The term “mishandled Botox” refers to any instance where botulinum toxin is used, stored, or administered improperly. This can include:

  1. Counterfeit Products: The use of fake or unapproved botulinum toxin, which may not meet safety standards.
  2. Improper Storage: Botulinum toxin requires specific storage conditions, such as controlled temperatures, to maintain its safety and effectiveness.
  3. Unqualified Administrators: Injections performed by individuals who are not licensed or adequately trained, which can lead to incorrect dosages or contaminated procedures.
  4. Unregulated Sources: Purchasing botulinum toxin from unauthorized suppliers or online platforms instead of FDA-approved vendors.

These mishandling practices increase the risk of complications and can result in the toxin spreading beyond the intended injection site, leading to systemic effects.

How to Stay Safe

For medspa owners, it’s critical to:

  • Source botulinum toxin only from FDA-approved vendors
  • Follow all storage and handling protocols to ensure product safety.
  • Verify that staff administering injections are properly licensed and trained.
  • Educate clients about the importance of receiving injections from reputable providers.

How to Build Trust with Potential Clients

To assure clients that your practice is reputable and safe:

  • Highlight certifications and licenses prominently in your office and online.
  • Share educational content about botulinum toxin safety and your commitment to quality.
  • Provide transparent information about the sources of your botulinum toxin products and their FDA approval.
  • Encourage client reviews and testimonials to build social proof.
  • Offer consultations where clients can ask questions and learn more about your safety practices.

The CDC’s investigation highlights the need for vigilance in the aesthetic industry. Medspa owners and practitioners must prioritize safety to prevent adverse outcomes. By adhering to regulations and building trust with clients, the industry can continue to provide these treatments safely and effectively.
